At last! A fearless dentist hero! Check out the March 30, 2009 New Yorker Magazine article “Tails of Manhattan” by Woody Allen. In this comedy, Abe Moscowitz, a “decent citizen, a dentist, a mensch,” dies of a heart attack and is reincarnated as a lobster. Not a very dignified beginning for the dentist, and I read on, expecting the usual “dentist as schlemiel” story. But what a surprise! Turns out that Moscowitz lost his life savings in the Bernard Madoff swindle and has a myocardial infarction when he finds out. So when Madoff turns up at the restaurant where the reincarnated Moscowitz is swimming around in a lobster tank, Moscowitz and a companion manage to turn over the tank and attack Madoff. Moscowitz grabs Madoff’s nose with his giant pincers and the swindler is severely injured. Moscowitz escapes to live out his lobster life in the waters of Sheepshead Bay. What a hero!

You probably already know that dark foods such as red wine, grape juice and berries can stain your teeth. But a study done recently at NYU demonstrated that white wine could also contribute to darker teeth. How so? Well, it turns out that it is not only the dark pigment in foods that stains teeth, but it is the acid in the wine that roughens and erodes the enamel. The eroded enamel holds a stain more readily than smooth enamel. Other acidic drinks such as sodas will also erode the enamel and thus contribute to stained teeth. Teeth whitening products may be a quick fix but not a long-lasting solution. Diet modification is in order!

I wrote an entry this week about sibling teeth on the Sibling Blog at www.siblingstuff.blogspot.com. Check it out! Sibling teeth are inherited, and the form and eruption patterns of sibling teeth can be remarkably similar. For example, if one of your children has over retained primary teeth, it is likely that her siblings may also have a delayed eruption pattern. If one child is missing one or more permanent teeth, his siblings may also be missing the same - or mirror - tooth.
But habits also affect the teeth. Thumb sucking, tongue thrusts, blanket sucking can all affect the development of the jaw bones. My patient Melissa had a habit of sucking her middle three fingers but her parents sent her for early orthodontic intervention. She is now a young woman with a lovely smile. He older brother did not have a finger sucking habit but he was older when he had orthodontic treatment and his smile is not quite so beautiful. Recently a retired Vietnam Vet came to my Long Island dental office with a complaint that the four implants his dentist placed under his denture several months ago had either fallen out or were loose. The loose implants were very painful. This had been going on for months with no resolution offered by his dentist. I immediately sent the patient over to the periodontist to remove the loose implants. The patient then came back to my office and I added my special tissue conditioner to his old denture. What a relief! He was comfortable and able to function until I could finish making him a new denture.
I explained to the patient that he had a very good residual ridge, and I didn’t think he needed the implants. What he needed was a denture that fitted and functioned well. His old dentist had added the implants to his worn out and ill-fitting denture. What was the point of that? A patient came to our Long Island dental office today to report that Good Morning America had a segment on a possible link between denture cream and imbalance. The source was a June 4, 2008 article in the journal Neurology which concluded that chronic, excess zinc intake can result in copper deficiency and profound neurologic disease. The authors identified four patients with various neurologic abnormalities. Each of these patients wore dentures and used more than two tubes of denture cream a week.
This is excessive!! If you must use denture cream, use just three to four dime-size dots in key places on the denture. You are using too much denture cream if the cream oozes out of the side of the denture when you put it in place.
But the real question here is why are these patients using denture cream? A denture that needs cream is a failing denture. My denture patients never use adhesives. I make the best dentures, and you don’t have to add glue!

Do you suffer from painful mouth sores, commonly called canker sores, and which dentists call aphthous ulcers? Well, you are not alone. Up to 25% of the population experiences canker sores at one time or another, and we see canker sores frequently in our Suffolk County Dental office. The exact cause of aphthous ulcers is unknown but stress, lack of sleep, trauma to the teeth or mouth, or allergies may all trigger an ulcer, which is a break in the mucous membrane. There are various over-the-counter remedies, but no effective long-term treatment exists.
However a recent study demonstrated that one sublingual tablet per day of vitamin B12 (1,000 mcg) resulted in a significant reduction of aphthous ulcers. Painful ulcers can be soothed by avoidance of spicy food, rinsing with salt water or mouthwash, proper oral hygiene and non-prescription local anesthetic ointments.

A recent dental journal article stated that “Now that the teeth are looking much better aesthetically, isn’t it time to consider the perioral areas around the smile? If the teeth look great but we ignore the rest of the face, then we have really limited what we can accomplish with aesthetic dentistry.” The author goes on to advocate that dentists learn to place dermal fillers to plump out lips and smooth out facial wrinkles around the mouth. The author concludes his article by stating that the dentist is the “healthcare professional of choice” to deliver such procedures.
So what do you think? I think this is pushing the envelope too far (an apt metaphor here). The American Dental Association defines dentistry as “the evaluation, diagnosis, prevention and/or treatment (nonsurgical, surgical or related procedures) of diseases, disorders and/or conditions of the oral cavity, maxillofacial area and/or the adjacent and associated structures and their impact on the human body; provided by a dentist, within the scope of his/her education, training and experience, in accordance with the ethics of the profession and applicable law.”
What’s interesting is that this definition of dentistry is vague enough to include the placement of dermal fillers by dentists. But it also should be noted that the statement was adopted by the ADA House of Delegates in 1997. The dental profession has certainly changed a lot since then! An updated definition is needed. For the present, this Long Island dentist will keep her distance from dermal fillers, and she will focus on the teeth and their supporting structures – where I believe the dental focus should be.

This week the Huntington Hilton hosted the Greater Long Island Dental Meeting. I spent all day Tuesday at the meeting, talking to dental suppliers about materials we use in my Suffolk County dental office and learning about new dental materials available. Talking to the suppliers is a great learning opportunity for interested dentists. I also attended one of the many courses offered. I take many days of continuing dental education each year so I can keep up with current techniques, materials and technologies.
I am very excited that I will be taking a year-long course on advanced dental restoration next year at the New York University School of Dental Medicine. I will be treating patients at the school’s clinic under supervision of top dental clinicians, and I will be attending lectures on advanced dental restoration and cosmetic dentistry, including implants, veneers, crowns, bridges and dentures. The course meets one day a week for 28 weeks. This will better enable me to offer dental treatment options for the increasing numbers of patients who come to my office with severely broken-down dentitions.

Baby boomers are very interested in staying young – and this includes keeping their teeth. A study of patients 65 of age and older showed that it was “critically important” to them to keep their natural teeth. Patients in the study who had 25 or more natural teeth (a full complement is 28, or 32 including the third molars or wisdom teeth) rated their dental health and quality of life better than people who had lost 4 or more teeth.
I am one Long Island dentist who understands that there is an emotional and psychological impact of tooth loss. I am sensitive to a patient’s feelings of embarrassment or vulnerability. Go for the second opinion. Don’t be embarrassed. People often call our Long Island dental office to ask for a second opinion. I am happy to talk to them, find out what their needs are and make suggestions. It’s your dental health and your pocketbook so you need to get as much information as you can, and you need to find a dental practitioner you can trust. Too many times people come to the office and have been told things that I think would not be in their best interest.
For example, a young woman came into my dental office last week and said that her front tooth had chipped and she was told she needed a crown. She was upset at the thought of putting a crown on her front tooth. I looked at the chip and told her that it should be repaired with a simple and undetectable tooth-colored composite bonding procedure. We took care of the chip and she was thrilled!
Another woman came into the office this week with a temporary bridge that had fallen out. She was told that one of the abutment teeth would have to be extracted and two implants placed. She was upset at losing another tooth. I took a look and recommended some gum work to save the tooth and placement of a permanent bridge. She was thrilled!
In both of these cases my recommendation was an alternative to what the patient didn’t want – the crown in the first case and extraction and implants in the second case. And my recommendation was considerably less expensive and a better service than what had been suggested at another dental office. So go for that second opinion.
